Total IgE & Allergy Evaluation

Total IgE measures the overall concentration of immunoglobulin E in the blood. IgE plays an important role in allergic reactions, and elevated levels can occur in atopic diseases, allergic conditions, certain parasitic infections, and some immunological disorders.

Total IgE is not a specific test for allergy. Specific-IgE testing and clinical history may provide more useful information when a particular allergen is suspected.

At National Path Lab (NPL), total IgE testing is available, typically used alongside specific-IgE testing and clinical assessment for a more complete allergy evaluation.

About Total IgE & Allergy Evaluation

Causes & Risk Factors

Total IgE testing is considered as a component of broader allergy or immune evaluation, rather than in response to specific risk factors:

•  Suspected atopic disease (eczema, asthma, allergic rhinitis) where overall IgE level may provide supportive information.

•  Unexplained allergic-type symptoms where a broader immune picture is helpful.

•  Suspected parasitic infection, which can also elevate total IgE.

•  Monitoring of certain atopic conditions over time, in specific clinical contexts.

•  As part of a broader immunological work-up when other immune disorders are being considered.

Symptoms & When to Test

Total IgE testing is not directly tied to a specific symptom, since it is a general marker rather than an allergen-specific test.

•  Considered alongside symptoms of atopic disease — eczema, asthma, allergic rhinitis

•  May be tested when allergy symptoms are present but the specific trigger is unclear

•  Used as part of a broader work-up rather than as a stand-alone diagnostic symptom-driven test

When Should You Consider Testing?

Total IgE is generally tested as part of a broader allergy or immune evaluation, alongside specific-IgE testing and clinical history, rather than as a stand-alone test for a specific symptom.

Why Tests Are Combined

Because total IgE can be elevated in many different conditions — atopic disease, parasitic infection, certain immune disorders — an elevated result alone does not identify the cause, which is why it is generally interpreted alongside specific-IgE testing (which identifies the specific allergen) and clinical history.

A normal total IgE does not exclude allergy, and an elevated total IgE does not confirm allergy — this is why total IgE functions best as a supportive, contextual piece of information rather than a stand-alone diagnostic test.

In practice, doctors often order total IgE together with a relevant specific-IgE panel, using the total IgE as background context for interpreting the more targeted specific-IgE results.

Total IgE

Measures overall immunoglobulin E concentration in blood; used as supportive information alongside specific-IgE testing and clinical history.

Most allergy and immunology blood tests do not require fasting. However, some tests ordered at the same time may have specific preparation requirements. Please follow the instructions provided by your doctor or NPL.

When to See a Doctor

•  Chronic or unexplained allergic-type symptoms

•  Known atopic disease (eczema, asthma, allergic rhinitis) requiring evaluation

•  Unexplained elevated IgE found on prior testing

•  As part of a broader immune evaluation recommended by your doctor

Frequently Asked Questions

No. Total IgE measures overall IgE levels, which can be elevated for many reasons, while specific-IgE testing identifies sensitization to particular allergens. They provide complementary, not identical, information.
Yes. A normal total IgE does not exclude allergic disease, which is why specific-IgE testing and clinical history remain important alongside total IgE.
No, this blood test does not require fasting.
Certain parasitic infections and some immunological disorders can also elevate total IgE, which is part of why it is not considered a specific allergy test on its own.
This depends on your clinical situation. Your doctor can advise whether total IgE, a targeted specific-IgE panel, or both together are most appropriate for your symptoms.
